Options for Independent Living
Assessed in-home support for adults 60+, or ages 18–59 with disabilities, living in a private home.
Service
Assessed support may include personal care, homemaking, meals, medical transportation, safety equipment, chores, and case management.
Eligibility
Cuyahoga residents age 60 or older, or adults 18–59 with disabilities, living in a private home or apartment. Need for daily-living help, financial circumstances, and ineligibility for Medicaid waiver services are considered. Licensed facilities are excluded.
