Start with what the client can actually use today. These questions are practice notes, not extra program rules. Confirm today’s hours, last arrival, household or ZIP eligibility, documents, appointment, walk-in capacity, proxy form, accessibility, and quantity before anyone travels.
- No kitchen or refrigeration: choose a prepared meal or request ready-to-eat items. Ask about dietary restrictions, chewing or swallowing needs, and a safe way to store food.
- No transportation or cannot carry groceries: screen for delivery, an authorized proxy, a nearby meal site, or a pantry reachable by transit. May Dugan publishes proxy pickup. Lakewood Community Services Center and Benjamin Rose have distinct delivery programs.
- No ID or permanent address: ask about alternatives before ruling out help. St. Vincent de Paul publishes no-ID access but also lists service boundaries. May Dugan’s no-ID policy applies to its clothing room. Do not assume it applies to food.

Transportation beyond the regular bus
Match the ride to the client’s municipality, mobility needs, and trip purpose before scheduling. For an insured client, use the transportation or member-services contact on the current plan card. Ask whether the trip is covered, who books it, how much notice is required, and whether wheelchair transport or an attendant is authorized. Get a trip number and return-ride instructions. A medical ride benefit should not be assumed to cover grocery trips. Also review RTA paratransit, Options for Independent Living, and aging-service entries.

Advocate intake checklist
Use this during a warm handoff. These are suggested questions, not universal eligibility requirements.
- Match the household: adult age, ZIP code, pregnancy or postpartum status, family composition, dependents’ ages, disability and access needs, and preferred language. Ask about gender-appropriate placement without assuming a provider’s policy.
- Confirm the actual service: Are you accepting new clients today? Is there a bed, appointment, or waitlist? What is the treatment level? Is medical clearance required? Who can make the referral? Get the intake worker’s name and a direct callback.
- Confirm costs and documents: coverage, uninsured funding, fees or deposit, ID alternatives, proof of residence or income, clinical records, a release of information, and court documents. Do not delay urgent screening only because a client lacks paperwork.
- Make placement workable: transportation, mobility or personal-care support, interpretation, service animals, dietary needs, belongings, medication storage, and continuation of prescribed medications, including medications for opioid use disorder. Ask about childcare and pregnancy care coordination.
- Close the loop: with consent, make a warm transfer. Record the service, date, contact, eligibility decision, next step, arrival instructions, and a backup referral. Use a safe callback method and share only necessary information.
